Executive Summary
The NOFO supports CDC’s efforts to protect U.S. residents traveling internationally. The goal is to help U.S. travelers with pre-travel preparation, access to timely health information while traveling, and timely research on health conditions.

The project will:

• Improve access to high quality travel preparation information for both healthcare providers and travelers.
• Collect health information before, during, and after travel through innovative electronic medical record (EMR), web, and mobile phone applications.
• Build on this platform to conduct timely research on emerging health threats, including:
  • Enrollment of prospective cohorts.
  • Developing and evaluating advanced diagnostics.
  • Cost-effectiveness and optimization evaluations.

This NOFO will support research and surveillance activities in a network of clinics that provide pre-travel health services to people with travel destinations outside of the United States, focusing on travelers at high risk for illness or injury while travelling.

During pre-travel visits, this system will anonymously collect specific data on travelers visiting international destinations. This NOFO will encourage high-risk travelers to seek pre-travel health consultations from a clinician to prepare them for a healthy return.

This will also contribute to the travel medicine scientific evidence base.

Examples of contributions may include:

• Prospective traveler cohort studies.
• Use of novel diagnostic techniques to estimate risks to travelers.

Many international travelers see a primary care clinician or another medical specialist for pre-travel consultations instead of a clinician who specializes intravel medicine.

This NOFO will also increase the awareness of travel medicine among primary care providers and provide them with appropriate tools in advising international travelers with up-to-date recommendations.

The focus of this notice of funding opportunity (NOFO) is to produce insights into disease detection and prevention strategies, burden, severity, diagnostics, treatment, and outcomes of infectious diseases in international travelers. Timely surveillance and sentinel detection are essential for collecting actionable data.

The research should:

• Address public health questions that improve the speed of outbreak detection.
• Help develop guidelines and recommendations to improve patient care.
• Enhance understanding of the natural history of disease.
• Optimize genomic surveillance as well as diagnostic and treatment strategies.
• Advance the global health security of the United States.
• This NOFO will have two components:
  • Component A – Clinic-based sentinel network.
  • Component B – Sentinel detection in international transportation hubs.

This funding opportunity will help strengthen sustainable systems that countries use to identify and control fungal disease threats. Activities supported may include: 

• Strengthening laboratory and diagnostic capacity.

• Improving data collection, analysis, and use to guide public health action. 

Projects are expected to: 

• Build on existing national and regional public health structures. 
• Promote practical approaches that countries can sustain and integrate into broader communicable disease and public health programs. 

Ideally, this work will strengthen national and regional capacity to detect, characterize, and respond to fungal diseases and improve coordination across sectors and borders. 

Strengthened systems will help countries better understand fungal disease patterns, identify emerging threats earlier, and carry out public health interventions quickly and effectively.

Summary 
This Notice of Funding Opportunity (NOFO) supports strengthening country systems to detect, notify, and respond to outbreaks by applying time-bound, evidence-based frameworks, such as the 7-1-7 target, which aims to detect outbreaks within 7 days, notify within 1 day, and respond within 7 days. This target was selected to promote rapid containment and minimize health and economic impacts. 

Activities under this award will be conducted outside the United States and its territories. Building on previous outbreak experience and other international health emergencies, this NOFO advances American interests by reinforcing systems that protect both the United States and recipient nations. 

Activities focus on improving early detection of biological threats, timely notification across sectors and borders, and effective response to contain outbreaks before they spread, consistent with the Detect, Notify, Respond (D- N-R) framework. Efforts include strengthening surveillance, expanding laboratory readiness, enhancing information flows, and improving response operations to reduce economic effects and improve health. These activities emphasize burden sharing, accountability, and efficiency to ensure benefits to Americans at home and abroad, while helping recipient nations achieve long-term self-reliance and stability.

Summary 
This notice of funding opportunity (NOFO) will enhance capacity to contain outbreaks through partnerships with Ministries of Health (MOHs), external stakeholders, and other governmental institutions. This NOFO prioritizes durable Global Health Security (GHS) programs to keep Americans safer, stronger, and more prosperous by improving public health globally. It applies lessons learned from recent significant public health events, including outbreaks of viral hemorrhagic fevers (VHF), highly pathogenic strains of influenza, and the COVID-19 pandemic. 

As global leaders in strengthening GHS systems, the U.S. government (USG) will use updated frameworks to advance disease detection, notification, and response strategies. This NOFO’s main outcomes are:

• Use time-bound evidence based frameworks, such as the 7-1-7 framework, to detect and respond to outbreaks more quickly for diseases that threaten global health security and the health of Americans at home and abroad.

Summary
In collaboration with U.S. Department of Health and Human Services (HHS), the Centers for Disease Control and Prevention (CDC), state and federally funded programs, you will implement a project focused on reducing chronic disease rates within a defined geographic area within four communities: Lake County, Indiana; Sandusky and Toledo metro area, Ohio; Brownsville, Texas; Petersburg, Virginia. The goal is to demonstrate how individual and system- level interventions can encourage healthier habits and choices that can reduce the burden of chronic disease in children. When these lifestyle and community changes are maintained over time, reductions in the burden of chronic diseases among adults in the United States may eventually result. Findings from these demonstration programs will inform the potential replication of successful interventions in additional communities. 

Additionally, HHS will use findings from this demonstration project to inform future approaches to funding and measuring effects of novel efforts to improve health for individuals and communities.

Summary 
The purpose of this Notice of Funding Opportunity (NOFO) is to decrease the impact of specific parasitic infections in the U.S. with public health practices and disease control strategies. The NOFO focuses on three parasitic infection areas: 
• Chagas disease.
• Soil-transmitted helminths. 
• Other parasitic diseases of domestic concern in the U.S. 

This NOFO will continue CDC’s Parasitic Disease Branch work to help healthcare providers (HCPs) better understand, detect, treat, and prevent parasitic infections. Improved understanding and management of parasitic diseases in the U.S. will help make Americans stronger, safer, and more prosperous. There are three components of this NOFO. You may apply for only one: 
• Component A will improve HCPs’ knowledge and practices. Increase testing and management of people with or at risk of Chagas disease. Expand the use of best practices, training tools, and materials. 
• Component B will strengthen public health efforts to monitor and control soil-transmitted helminth infections (like hookworm). It focuses on areas in the Southeastern U.S. (as defined by the U.S. Census Bureau) that are historically affected. 
• Component C will target the prevention, control, health education, and, where applicable, elimination of other parasitic diseases in the U.S. Diseases include cyclosporiasis, cysticercosis, echinococcosis, leishmaniasis, lymphatic filariasis, and toxoplasmosis.

We will not accept proposals focused on malaria, amoebiasis,cryptosporidiosis, giardiasis, or any international activities. Your strategies should aim for significant public health benefits and reach a large portion of at-risk populations.

This NOFO focuses on protecting Americans from global health threats across three strategic areas: • Strengthening Early Warning Surveillance.
