Report an Adverse or Unanticipated Event

As per IACUC Guidance 115, individuals must notify the IACUC regarding any:

  • unanticipated deaths or clinical signs
  • adverse events
  • unexpected study results, or
  • phenotype that negatively impacts the welfare of an animal

Notify the IACUC using the Reportable New Information Form

Notification must occur even if the issue is reported to a University Animal Care veterinarian, and/or the issue has been corrected.

Notification must be timely, to minimize the adverse effect on animal welfare. Email completed forms to: orcr-iacuc@email.arizona.edu
